The following changes will be made to your current dental plan. Please review the documents listed below.

For members and dependants not currently undergoing ortho treatment: you don't need to take any further action. You will automatically be enrolled in the Healthplex Indemnity Plan (PPO) effective July 1, 2024.

Superior Officers Council (Not Continuing Ortho)

For member/dependants currently undergoing ortho treatment:
You will continue under the SOC Buy-up option until your ortho treatment is completed. Once completed, you will automatically be enrolled in the Healthplex Indemnity Dental Plan (PPO).

To locate a participating provider in your area please use the form corresponding to your plan type. A Healthplex Customer Service Representative is available to assist you Monday through Friday 8:00 a.m. to 6:00 p.m. at 1-800-468-0600. As of your effective date, you can use the Healthplex website to view and verify your account information and to request additional identification cards.
