With the CapDent Dental Insurance plan, you select a primary care dentist when you enroll. You must receive treatment from your assigned primary dentist who will refer you to a specialist for specialist services. Specialist services may require Pre-authorization.

Changing My Dentist


You can change your dentist after you sign up. You need to call Customer Service at 1-800-468-0600. Just send your request in writing by submitting the Subscriber Change Form by fax or email. Click here for the Subscriber Change Form.

Changes received by the 15th day of the month will be effective the first of the current month.

Note that if you have treatment in progress, such as dentures or crowns that have been prepared, you must have the service completed before you change to another dentist.
