1. What type of dental benefit plans are available through Healthplex?
   Healthplex offers many types of insured dental benefit plans. The following are examples of programs that are underwritten by Dentcare Delivery Systems, Inc., International Healthcare Services, Inc., Healthplex Insurance Company:
          Managed Care Plans for Small and Large Groups.
          Reimbursement (Fee-For-Service) Plans for Small and Large Groups.
          Managed Care Plans for Individuals
          Voluntary, Payroll Deduction Plans for Small Groups.
   
2. Does Healthplex have a Preferred Provider Organization (PPO)?
   Healthplex has developed a large PPO that includes general dentists as well as specialists. Enrollees with insured reimbursement plans may receive services from any participating dentist in order to reduce their out-of-pocket expenses.
   
3. What if my client would like to self-insure their dental program?
   Healthplex is a leader in dental plan administration and provides a wide range of services for self-funded groups. The company can help design a program or can simply process claims for those in need of our expertise.
   
4. Can I receive commission for selling one of your dental plans?

5. How do I become a "General Agent" for Healthplex?
   In some cases, general agency status is available to large producers. If your company has a network of sub-agents and you have placed a significant amount of business with our company, you may be entitled to additional compensation. 6. How do I submit a case for underwriting?
   All that's needed is a Group Application along with the Individual Enrollment Forms and the first month's premium payment. For your first case, please submit a copy of your license, Broker Registration Form and W9.
   
7. How can I obtain brochures, group applications and other enrollment materials?
