Brokers looking for a quote from underwriting must submit a census electronically in Microsoft Excel format to Groupservices@Healthplex.com.

The census should include all covered employees (with noted spousal waivers) along with the gender, occupation, and current and renewal rates. In addition, we would need the current benefit plan, length of time with benefits, and the employee contributions.

Depending upon the plan selected, minimum participation requirements may apply.
