* Once you enroll, rates are locked in for 12 months. Dental Premiums and Club Agency expenses are included in this cost. Enrollment periods begin the first of each month. You must enroll by the 20th of any given month for your coverage to begin on the 1st of the subsequent month.

** Enrollment in this program is on an annual basis; you must sign up for 12 months.

Payments can be made on a monthly or annual basis with your credit card.
