How Our Plan Works

Once you enroll in the Club "A" Dental Plan, simply wait for your ID card to arrive and make an appointment with any of our participating dentists. All you need to do is simply present your ID card at the time of your visit and discuss your treatment plan with your Dentist.

Then your Dentist submits a Claim Form for prompt payment to Healthplex, Inc.

There are no payments for most diagnostic and preventive services, most fillings*, and anterior (front) root canals*. For all other procedures you pay the Dentist the fees listed on the Schedule of Member Payments.

Any service that is not covered by the plan can be rendered by any participating provider at the reduced Healthplex PPO Schedule of Member Payments. Perhaps one of the best parts of this plan is that you have complete predictability with no surprises.

Our Dental Plan:
Our dental plan is insured by Dentcare Delivery Systems, Inc., which is licensed by the NYS Dept of Insurance. All participating providers are credentialed by Healthplex which is certified as a Credentials Verification Organization (CVO) in ten out of ten credentialing elements by the National Committee for Quality Assurance*. Achieving CVO certification from NCQA demonstrates that Healthplex has the systems, process and personnel in place to thoroughly and accurately verify providers’ credentials and help health plan clients meet their accreditation goals.

Our Dental Network:
Members can access a network of over 3,500 dentists in the metropolitan area. All participating providers have been carefully screened and must meet rigid professional requirements such as proper credentials, office cleanliness, commitment to prevention, modern equipment, standardized infection controls and sufficient and qualified staff.
