Costs

• Once you enroll, rates are locked in for 12 months. Dental Premiums and Club Agency expenses are included in this cost. Enrollment periods begin the first of each month. You must enroll by the 15th of any given month for your coverage to begin on the 1st of the subsequent month.
• There is an extra fee of $120/year for each family member in excess of five (family membership includes member plus four dependents).
• Enrollment in this program is on an annual basis; you must sign up for 12 months.
