Renewal & Cancellation Policy

I agree to maintain my enrollment for a minimum period of one year, unless I terminate coverage within 10 days of my effective date. I understand that I can terminate at any time during my one year enrollment period if I am not satisfied with the participating dentists or if the plan is unable to provide me with covered services. In this event, I will be entitled to a prorated return of my annual premium, less a $25 termination fee.

I agree to automatically renew coverage on each anniversary date unless I cancel with 30 days prior notice. Authorization of credit card payment will continue until cancelled by me in writing.
