Billing Terms:
- Eligibility You must be an active member of AAA to qualify and remain eligible for coverage. If you end your membership with AAA, your dental coverage is subject to cancellation.
- Payment Information You must pay for this dental benefit with a valid credit card. You may elect to pay monthly or annually. This is a pre-paid dental insurance plan. If you choose the monthly option, your credit card will be charged up to 14 days prior to the commencement of the ensuing monthly coverage period. Whether you select monthly or annual premium payment, Healthplex will continue to charge your account for all future premiums following your enrollment until such time you notify Dentcare or Healthplex that you wish to terminate your policy. Note that enrollment in this program is on an annual basis so you may not terminate the policy until twelve months from the date you enroll. Declined credit card transactions are subject to a $25 processing fee and will result in a cancellation of coverage.
- Enrollment Periods Once you enroll, rates are locked in for 12 months. Dental Premiums and Club Agency expenses are included in this cost. Enrollment periods begin the first of each month. You must enroll by the 20th of any given month for your coverage to begin on the 1st of the subsequent month.
- Non-Guarantee of Payment Enrollment and payment of premium in the online enrollment application is not a guarantee of claim payment. To be covered, services must be in accordance with Dentcare's treatment guidelines. All services must be performed at a participating dental office and the patient must be covered by Dentcare for the day services are completed.
- Refunds There are no refunds of premium paid for this coverage. If you cancel your coverage, your cancellation date will be effective on the last day of the period of your most recent payment.
- Renewal of Coverage Your coverage period is from your coverage start date until, at a minimum, 12 months from that date. Your coverage is automatically renewed after 12 months unless you notify Dentcare or Healthplex online or by submitting a termination of coverage letter.
- Cancellation of Coverage To cancel coverage, you must notify us online or by submitting a termination of coverage letter. Letters should be mailed to Club "A" Dental Plan Enrollment Department. 333 Earle Ovington Blvd. Suite 300 Uniondale, NY 11553-3606. Online notifications should be sent to clubadental@healthplex.com. Cancellation of coverage is effective on the last day of the period of your most recent payment. Dentcare reserves the right to terminate coverage after appropriate notification due to non-payment of premium. If you choose to cancel coverage after one year, you may purchase coverage as a new subscriber with a new effective date twelve months after canceling coverage. The waiting period for increased benefits begins again with the new effective date. No reinstatement of coverage is allowed.
- Guarantee of Rates Rates are guaranteed for the 12 months following your enrollment. Sixty (60) days prior to the end of your coverage period, Healthplex will email a notification to you indicating any change in rates.
- Consent By enrolling online and in making a claim for dental benefits, you agree on behalf of yourself and your covered dependents, if any, that providers may release to Dentcare and Healthplex, and that they may use and disclose, your individually identifiable health information in accordance with their Notice of Privacy Practices.
- Authorization I certify that all information submitted to Dentcare and Healthplex is true and correct to the best of my knowledge. I understand that the effective date and cancellation date of my insurance coverage will be determined by Dentcare in accordance with the underwriting guidelines associated with this plan. I authorize Healthplex to charge my credit card for the indicated payment frequency. I understand that if credit lines are not available, my coverage will be subject to termination after appropriate notification.
