This plan is available on a voluntary basis to groups with a minimum enrollment
of three. It is a fee-for-service reimbursement plan and includes coverage for
services rendered by any dentist. Enrollees have the option of reducing their
out-of-pocket expenses by seeking care from a Healthplex PPO provider. These
dentists have reduced their fees and will only charge patients the difference
between plan allowances and the PPO scheduled amounts. The plan has a
deductible of $40, and an annual maximum of $1,200 and requires the use of
claim forms for reimbursement. There are waiting periods of 12 months for
prosthetics and 24 months for orthodontic coverage. A discount Vision Plan from
Davis Vision is included for all enrollees.
There is an additional monthly
premium for each family member in excess of five. Premiums are billed
monthly as follows:
