OMNI Group Plan (New York Only)
Underwritten by Dentcare Delivery Systems, Inc.
This plan is available to groups with a 50% participation requirement (minimum
of 3 lives). It is a fee-for-service plan. The plan has a $50 deductible ($150
family) and pays percentages of scheduled fees for all covered services: 100%
for diagnostic and preventive care, 80% for basic restorative, endodontic and
surgical care, and 50% for major restorative, periodontal, and prosthetic repair
services. Benefits are subject to an annual maximum of $1,250 and services can
be rendered by any dentist. Enrollees who are treated by Healthplex PPO
providers will have significantly lower out-of-pocket expenses. A discount
Vision Plan from Davis Vision is included for all enrollees.
There is an
additional monthly premium for each family member in excess of five.
