This benefit can be be purchased by groups with as few as two employees. For
more information on our Low and Medium Options, please contact one of our
benefit specialists. Two plans that are available are the High Option and the High
Option with Enhanced Orthodontics. All enrollees in a given group must select
the same option. Both plans have minimal copayments (that you pay directly to
your participating dentists) and have no deductibles, maximums, waiting periods
or limitations on pre-existing conditions. Coverage utilizes the CapDent network
of participating dentists. There is no Out-of-Network benefit in this program.
The enrollee’s managed care general dentists provide all general dentistry.
Specialty care can be provided by CapDent Specialists (at a 25% discount from
usual fees) or by Healthplex affiliated specialists at fixed copayments (by referral
only). With the Enhanced Orthodontics option, 24 months of care are covered for
a fixed copayment of $1,200 ($1,600 for adults).
There is an addtional
monthly premium for each family member in excess of five. Premiums are
