CapDent Plus Ultra Plan (New York Only)

This plan is available on a voluntary basis to groups with a minimum enrollment
of three. It is a point-of-service plan and includes services rendered by any
dentist. The In-Network option is the same as in the regular Capdent Plus Plan,
but the Out-of-Network benefit is enhanced: scheduled reimbursements are 20%
higher, waiting periods are reduced to 9 months for prosthetics and 18 months for
orthodontics, and the orthodontic benefit has been extended to 30 months with a
$900 lifetime maximum. In addition, all Ultra enrollees are covered by Davis
Vision for a $10.00 eye examination as well as the standard discount coverage
included in the regular CapDent Plus Plan.
There is an additional monthly
premium for each family member in excess of five. Premiums are billed
monthly as follows:
