CapDent PLUS PLAN (New Jersey)

This plan is available on a voluntary basis to groups with a minimum enrollment
of three. This is a dual choice program and includes coverage for services
rendered by In-Network dentists. The primary care plan is similar to CapDent
but has lower out-of-pocket expenses and a $5.00 per visit fee. The EPO plan has
a larger network of providers and includes a $40.00 deductible and an annual
maximum of $1,200 per person. A discount Vision Plan from Davis Vision is
included for all enrollees. Premiums are billed
monthly as follows:
